What the description leaves open: which settings qualify, how many preceptors you may use, and whether any hours can sit in specialty care. A 2017 Bradley webinar allowance put as many as 100 of the 750 FNP hours in specialty clinics such as cardiology, a natural match for chronic disease, but that allowance is dated, so confirm it in the packet.

What chronic care asks of a clinical site

Chronic disease is managed across visits, so the best NUR 677 site is one with a stable patient panel: people returning for medication checks, lab reviews and adjustments to a long-term plan. Bradley's 2017 FNP competency guide already counted follow-up visits among the visit types students should log, and this is the course where they take center stage.

The catalog keeps the lifespan framing, which matters when you compare sites. A family practice covers children, adults and older adults in one place. A practice that sees only adults may cover part of the course well, so ask course faculty whether an adult-only site is acceptable before a preceptor signs. Working the same weekdays each week also helps, because you meet the same patients on their return visits.

Keeping your NUR 676 preceptor, or moving on

Either choice can work, and each has a cost worth knowing before you ask.

Staying with the same preceptor. Bradley's 2017 preceptor webinar capped reuse of any one preceptor at twice. A reuse meant sending only the one-page initial preceptor form again, plus updated documents if the license, CV or malpractice copy had gone out of date.
Spending a use. Under that 2017 rule, a preceptor who covers both NUR 676 and NUR 677 has no uses left for NUR 678 or NUR 679.
Changing preceptors. Bradley's online program pages encourage a new preceptor whenever the clinical focus shifts, and a chronic-care practice may not be the clinic where you saw acute visits.

The 2017 limits came from the old five-practicum design, where at least three of five FNP preceptors had to be NPs. The current reuse and NP-share rules are in the FNP clinical packet, and the preceptor requirements page tracks what is public.

Packet timing when NUR 677 follows NUR 676

Bradley wants each DocuSign preceptor packet in hand by the middle of the term that comes before the clinical course. Here is how that plays out for the second practicum.

Find NUR 677's semester. Your program of study in MyOnline shows when each practicum is scheduled.
Count back to the prior midterm. If NUR 677 directly follows NUR 676, the deadline sits halfway through your first practicum.
Confirm 150 hours of room. Ask the preceptor which days they can host you in that term before any paperwork starts.
Assemble and send the packet. The preceptor's license and CV, a copy of their malpractice coverage, the practice address and the course all go into the DocuSign packet; the preceptor packet page lists the parts.
Finish setup before day one. Once Bradley approves, add the preceptor and site in Typhon so your first log has somewhere to go.

Telehealth follow-ups and the unpublished policy

Some chronic-care practices run part of their follow-up schedule by video. Before you count any of that time, know that Bradley's graduate handbook sends FNP and AG students to a telemedicine policy for this exact question, and that policy is not available to the public.

Ask course faculty how telehealth visits are treated in NUR 677 before the course starts, and record the answer. If your preceptor's schedule leans heavily on video visits, choose in-person days for your clinical time so every hour you log in Typhon is clearly countable. If faculty do allow telehealth time, ask whether those entries need flagging in Typhon and whether the course caps them. Write the answer down with the date and who gave it.

The October calendar math

The FNP welcome site's rule has two halves: finish the Nurse Practitioner Skills Workshop before NUR 679, and attend while one of the first three FNP practicums is on your schedule. The workshop happens once a year, over two October days in Peoria.

That makes NUR 677 a common window. If this course runs in a fall semester, attending that October keeps the trip out of NUR 678 entirely. If you skip it, the next October has to fall while you are still in NUR 678, or NUR 679 waits. Work it out against your program of study on the FNP bootcamp page and tell your preceptor early which two days you will be away, so your clinic weeks can be arranged around the trip.

It is a typo in the 2026-27 catalog. The course descriptions and program tables print the title as Primary Care Chronic Conditions Across the Lifespan II II. It is the same NUR 677, the second FNP practicum, and the handbook's FNP packet list names it simply as NUR677.

Can one preceptor cover both NUR 676 and NUR 677?

It can, under the 2017 webinar's two-course ceiling per preceptor, and the second course only needed a fresh copy of the short initial preceptor form. The catch is that this spends both uses, leaving that preceptor unavailable later. Look up the current limit in the FNP packet before you ask for a second term.
